TERMS & CONDITIONS
Interlinked Wellness LLC (“Interlinked Wellness,” “we,” “us,” or “our”) provides telehealth-based functional medicine services. By accessing this website or engaging with our services, you agree to these Terms & Conditions.
APPOINTMENTS & CANCELLATIONS
Appointments are scheduled in advance and reserved exclusively for you. Cancellations or rescheduling must be made at least 24 hours in advance. Missed appointments or cancellations made within 24 hours will be charged in full. If you arrive late, your session may be shortened accordingly.
FEES, PAYMENTS & REFUNDS
All services are prepaid unless otherwise specified. By purchasing any service, you authorize Interlinked Wellness to charge your selected payment method. All fees are non-refundable, except for the initial consultation under the condition outlined below. The initial consultation is eligible for a refund only if the patient attends the scheduled appointment in full. Failure to attend the appointment for any reason, including late arrival or no-show, voids eligibility for a refund. No refunds will be issued for any other services, including but not limited to lab testing, consultations beyond the initial visit, programs, or services that have been initiated, scheduled, or rendered. By completing a purchase, you acknowledge and agree to this no-refund policy without exception.
TELEHEALTH SERVICES
All services are delivered via telehealth in accordance with applicable laws. A provider-patient relationship is established only after a formal consultation with a licensed medical provider. By participating in telehealth services, you consent to receiving care remotely, understand the limitations of telehealth, and acknowledge that you may withdraw consent at any time without affecting your right to future care. You may request access to your medical records at any time.
ELECTRONIC COMMUNICATION
We may communicate with you via email, phone, or SMS for scheduling, administrative, and service-related purposes. These methods may not be secure, and you agree to use them at your discretion. Sensitive medical information should be shared through secure, HIPAA-compliant platforms. We do not guarantee immediate response. Do not use electronic communication for emergencies.
EMERGENCIES
Interlinked Wellness does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
NO GUARANTEE OF RESULTS
Individual results will vary. We do not guarantee specific outcomes from any service, treatment plan, or recommendation.
NO PROVIDER-PATIENT RELATIONSHIP VIA WEBSITE
Use of this website does not establish a provider-patient relationship. A provider-patient relationship is only established following a direct consultation with a licensed medical provider.
MINORS
Services for minors require consent from a parent or legal guardian. We will comply with applicable laws regarding parental access to information while maintaining appropriate confidentiality.
TERMINATION OF SERVICES
We reserve the right to discontinue services at our discretion, including but not limited to situations involving non-payment, misuse of services, or determination that services are not clinically appropriate. Where appropriate, referrals to other providers may be offered.
LIMITATION OF LIABILITY
To the fullest extent permitted by law, Interlinked Wellness shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services or website.
