Functional medicine and conventional medicine differ in what they ask, not in whether they are scientific. Conventional medicine asks which diagnosis fits and which treatment matches it, and it is extremely good at that. Functional medicine asks why physiology shifted, in what order, and what is holding it there. The two use overlapping tools, are practiced by the same categories of licensed clinician, and answer different questions. The comparison below is deliberately even-handed: it names where conventional care is clearly superior, where the functional model earns its keep, how mature the evidence actually is on each side, what each approach costs in money and time, and the safety rules that should govern any patient using both.

Where conventional medicine clearly wins

A comparison that cannot name the other side’s strengths is marketing. Conventional medicine is the correct and often the only correct answer in these situations, and no functional medicine practice should suggest otherwise.

  • Acute and emergency care. Chest pain, stroke symptoms, severe infection, trauma, appendicitis, obstetric emergencies. Speed and protocol beat root-cause reasoning every time.
  • Cancer diagnosis and treatment. Screening, staging, oncology, and surgical management sit entirely within conventional care.
  • Conditions with strong trial-backed treatments. Type 1 diabetes, hypothyroidism requiring replacement, epilepsy, severe asthma, HIV, autoimmune disease requiring immunosuppression. Withholding these treatments in favor of lifestyle change is dangerous.
  • Surgical and procedural problems. Fibroids causing severe bleeding, gallstones, hernias, joint replacement.
  • Population-level prevention. Vaccination, cancer screening, blood pressure and lipid management. These have mortality data behind them that no functional protocol can match.
  • Serious mental health conditions. Psychosis, bipolar disorder, severe depression, and suicidal ideation need psychiatric care, not a nutrition protocol.

There is also a structural strength worth naming: conventional medicine is regulated, audited, and standardized in a way that a diffuse clinical framework is not. Guidelines are revised publicly when evidence changes.

Where the functional model adds something real

The strain in conventional care is not clinical competence; it is fit. The 15-minute, one-problem-per-visit structure handles discrete problems well and multi-system chronic complaints poorly. That mismatch is where the root-cause model earns its place.

  • Symptoms that cross specialties. Fatigue plus bloating plus anxiety plus cycle change usually becomes three referrals and no synthesis. A single clinician holding the whole picture is a structural advantage, not a philosophical one.
  • Subclinical patterns. Thyroid antibodies present years before TSH moves; ferritin at the bottom of the range with symptomatic fatigue; fasting insulin rising while glucose stays normal. These are real physiology, and they usually fall between the diagnostic thresholds that trigger conventional action.
  • Perimenopause specifically. The transition produces sleep, mood, metabolic, cognitive, and cycle symptoms at once and remains under-taught in general practice.   • Time to take a proper history. Much of what a functional clinician finds is in the timeline, and a timeline takes an hour to construct.
  • Lifestyle interventions delivered as treatment. Nutrition, sleep, and stress physiology are evidence-based, but conventional visit structures rarely allow the time to implement and follow them properly.

Evidence maturity

This is where fair comparison matters most. Conventional treatments generally arrive with randomized controlled trials, systematic reviews, and regulatory oversight. Functional medicine as a whole model has far less of that, and anyone who tells you otherwise is overselling.

The strongest single study is a retrospective cohort analysis from the Cleveland Clinic Center for Functional Medicine published in JAMA Network Open, which compared patients treated in the functional medicine center with matched primary care patients and found larger improvements in PROMIS Global Physical Health scores at six months (Beidelschies et al., 2019). Three limitations deserve equal billing: it was observational rather than randomized, so selection effects cannot be excluded; it measured patient-reported quality of life rather than disease outcomes; and attrition at six months was substantial.

A more useful way to think about it is to separate the model from its components. The framework — matrix mapping, timeline history, systems thinking, taught by the Institute for Functional Medicine — has limited outcome evidence. Many of the interventions it uses are individually well supported: Mediterranean-pattern eating for cardiovascular risk, cognitive behavioral therapy for insomnia, resistance training for insulin sensitivity and bone density, vitamin D repletion in documented deficiency. The framework is a way of deciding which evidence-based lever to pull first, in what order, for this person. That is a defensible claim. "Functional medicine cures chronic disease" is not.

Testing: what each model orders

Both models use the same accredited laboratories for standard assays. The differences are breadth, how results inside the reference range are read, and the addition of specialty panels whose validation varies.

Two rules. Broad panels generate incidental abnormalities, so a result is a question rather than a finding until it is corroborated by symptoms and repeat testing. And a clinician should state which tests are well validated and which are exploratory, before you pay for them.

Credentials and regulation

Both models are practiced by the same license categories: physicians (MD, DO), nurse practitioners (APRN), and physician assistants. Functional medicine is not a separate license, which is why the underlying credential is the thing to verify. In Texas the Texas Medical Board regulates physicians and the delegation of medical acts, while the Texas Board of Nursing licenses advanced practice registered nurses. A Texas APRN can diagnose, order and interpret testing, and prescribe under a prescriptive authority agreement with a delegating physician, and Texas law explicitly recognizes telemedicine encounters. The practical test is the same in either model: an active state license, verifiable training, and a willingness to refer out. Cost and time compared

Conventional care is usually covered by insurance, with a copay or deductible, and the cost of the model is paid in fragmentation and waiting: multiple appointments, referral delays, repeated retelling of the same story. Functional medicine is usually paid out of pocket, most often with a superbill submitted for possible out-of-network reimbursement, and the cost is paid up front and visibly.

The time ledger runs the other way. A conventional workup for a chronic multi-system complaint can take a year of sequential referrals to reach a synthesis, or never reach one. A functional workup front-loads the time into one long first visit and a defined reassessment schedule. Neither is free. It is worth asking any practice what the consultation includes, whether a superbill is issued, which testing is billed separately, and whether supplements are sold in-house.

Integration, not replacement

The framing that survives scrutiny is not "which one is right" but "which one for which problem, and how do they communicate." The following composite illustration is not a patient record and not an outcomes claim; it shows how the two models divide the work in a typical midlife presentation.

Practical integration also means matching the intervention to the problem, which is what the treatments offered by a functional practice are for. Beyond that it means one shared record. Tell your primary care clinician what functional testing was run and what you are taking, including supplements, because interactions are real. Safety guardrails

Five rules protect a patient using both models at once.

  1. Red flags go conventional first, every time. Unexplained weight loss, a breast lump, postmenopausal bleeding, blood in stool, severe or new headache, chest pain, or a sudden neurological change need urgent conventional assessment. No root-cause workup substitutes.
  2. Never stop prescribed medication without the prescriber. Thyroid replacement, antidepressants, antihypertensives, and immunosuppressants all carry real risk on abrupt withdrawal.
  3. Disclose every supplement to every clinician. St John’s wort alters the metabolism of many drugs, high-dose vitamin K interacts with warfarin, and iodine can destabilise thyroid disease. Supplements are not automatically benign.
  4. Treat unrecognized diagnoses as a warning. "Adrenal fatigue" is not a recognized medical diagnosis and a systematic review found the evidence does not support it (Cadegiani and Kater, 2016). HPA-axis dysregulation, meaning measurable change in cortisol rhythm under sustained stress, is the defensible framing.
  5. Require a reassessment point. Any plan should state what will be measured and when it will be reviewed, so that a protocol that is not working can be stopped rather than extended.

FAQ

Is functional medicine evidence-based?

Partly. The answer distinguishes two things. Many individual interventions used in functional medicine have solid trial evidence, including dietary patterns, exercise prescriptions, sleep interventions, and correction of documented deficiencies. The overall model has much less outcome evidence, with the Cleveland Clinic cohort study in JAMA Network Open as the main published example and observational limitations attached. Some specialty tests used within the field are not validated for clinical decision-making, and a careful clinician says which are which.

Can I see a functional medicine clinician and keep my regular doctor?

Yes, and that is the arrangement most likely to work. Keep your primary care clinician for screening, acute care, and any diagnosed condition under treatment. Use the functional consult for the chronic pattern nobody has synthesized. Share test results in both directions and disclose every supplement, since interactions with prescribed medication are the main safety risk in running both at once.

Is functional medicine more expensive than conventional care?

Usually yes at the point of service, because most practices operate outside insurance networks and the visit lengths the model requires do not fit standard reimbursement. Many practices issue a superbill for possible out-of-network reimbursement, and health savings or flexible spending accounts can generally be used for qualifying medical expenses. Conventional labs ordered through a standard laboratory are often covered even when the consultation is not. The counterweight is the fragmentation cost of repeated specialist visits that never produce a synthesis.

Which one should I choose for perimenopause symptoms?

It is not an either-or. Menopausal hormone therapy is a conventional treatment with a substantial evidence base and a nuanced risk profile, and current guidance from The Menopause Society is the reference point for that decision (The Menopause Society). Sleep, insulin sensitivity, muscle mass, stress load, and nutrition all shape how the transition is experienced and are where a functional workup contributes. A clinician who will discuss both, without advocating for or against hormone therapy on principle, is the one to look for.
